Migraines can be more than just a pain in the head. Basilar migraines, once known as basilar artery migraines or BAMs, are an extraordinarily rare but potentially life-threatening variant of the classic migraine with aura.

Basilar migraine symptoms are caused by constriction of the basilar artery, which supplies blood to the brain stem. BAMs were originally thought to affect only young women and adolescent girls, but research shows that while they are primarily a problem for these groups they can occur in people of all ages and genders.

During the aura phase, basilar migraine symptoms may include loss of balance, double vision or partial vision loss, lack of coordination, numbness on one or both sides of the body, weakness, dizziness or confusion and severe vomiting. The symptoms typically last an hour or less and disappear when the headache begins, but may last as long as days after the headache pain has disappeared. Some basilar migraine sufferers pass out or lose consciousness during the aura phase as well. In extremely rare cases, they may even slip into a coma that can last hours or days.

The danger of basilar migraines is that they can lead to a transient ischemic attack (TIA) or stroke. A transient ischemic attack is essentially a miniature stroke resulting from a temporary interruption of the flow of blood to the brain. Unlike strokes, TIAs have not been shown to cause permanent damage to the brain and most neurological problems that arise from them, like slurred speech or weakness on one side, clear up within twenty-four hours of the attack.

The basilar artery is located at the back of head. The headache associated with basilar migraines is usually a severe throbbing ache on both sides of the back of the head, as opposed to the unilateral temple throbbing more commonly associated with migraines.

A migraine aura is a specific type of prodrome, or premonition, that heralds a migraine. It is a symptom or series of symptoms that let migraineurs, people who suffer from chronic migraines, know in advance that a migraine headache is about to make itself felt.

Aura effects develop over about five minutes and generally occur from twenty minutes to an hour prior to the start of a headache. They are a symptom of what used to be called “classic migraine”. Recently, medical practitioners have moved to a more scientifically accurate term, migraine-with-aura. Only about 15-20% of migraineurs consistently experience any kind of aura before a headache begins.

Aura Effects

Most migraine auras are visual. These auras are called positive visual phenomena because, rather than their vision fading or going dark, people who experience them see things. Examples include:

-Auras or Haloes: a nimbus of light or haze surrounding objects, particularly light sources

-Flashing or Floating Lights: colored or white (rarely dark) spots that appear to move

-Lightning Bolts: a jagged or zigzag line that sparkles and/or arcs over the field of vision; with the frequency or severity increasing over time until the person can no longer see (a whiteout); This effect usually stops with the onset of headache pain.

-Photophobia: an extreme sensitivity to light; this effect frequently lasts the duration of the headache

Most auras last less than an hour. Migraineurs who experience auras that last more than an hour are said to suffer from migraines with prolonged aura. In some cases a migraineur may experience aura effects without a headache developing, but it is still considered to be a symptom of chronic migraines. At various times the same person may experience all three variations, migraine with aura, migraine with prolonged aura, or typical aura without headache.

What actually happens in your body to cause headache?

The skull bones and brain tissues do not have pain sensitive nerve fibers. But muscles of face, mouth, throat, neck, scalp and blood vessels on the surface and base of brain are sensitive to pain as they contain pain sensitive nerve fibers.

The tip of these nerve fibers gets stimulated by various causes like stress, muscle tension, lack of sleep etc. The stimulated nerve tip sends message up the length of the nerve fiber to the related nerve cells of Brain. Brain locates the region of stimulated nerve tip based on these signals and we feel the pain or ache.

The headache may be mild, chronic or severe depending on the underlying cause and pain bearing capacity of the individual.
Our body produces natural pain killing proteins called endorphins. One of the theories suggests that people with low endorphin levels suffer from severe headache and other chronic pains in comparison with people having high endorphin levels.

The same causes of head ache have been explained in Ayurveda also. These causes vitiate all the three doshas (vata, pitta and kapha). The vitiated doshas cause tension, migraine, sinus and other head aches.

Few common types of headache are Tension headache, Migraine headache and sinus headache.

Tension headache: Tightening of muscles of face and neck cause tension headache. It is characterized by a dull ache in forehead or back of head and pain in neck and shoulders which travels to head. Causes like less sleep, stress, reading for a long time, and working in front of a computer for long hours can precipitate tension headache.

Migraine Headache: Migraine headache is caused when blood vessels of head get too much dilated or constricted. Females are more prone to this type of headache than males and migraine tends to run in families.

A person suffering from migraine headache feels like vomiting becomes too sensitive to light, sound and other activities. Few people experience spots or flashes before headache.

Few facts like menstruation in females, caffeine, alcohol, aged cheese, cured meat, stress, change in sleeping pattern, strenuous exercise trigger migraine headache.

Sinus Headaches

A sinus headache surfaces when fluids in the nose are not able to drain well and pressure builds up in sinuses. A cold, dust allergies, dirty or polluted water air travel may cause this type of headache.

Sinus headache causes pain in forehead, cheek bones, nose and stuffy nose. The headache increases when face is bent or touched. The symptoms are worse in the morning.

Although headaches are very common, It is recommended to consult a doctor about your headache. Go to an emergency department if any of the following symptoms occur:

• Pain that develops very rapidly

• A change in concentration or ability to think

• A change in level of alertness

• Altered speech

• Weakness, numbness, or difficulty in walking

For tension headaches massage few drops of peppermint oil or eucalyptus oil on temples.

Massage the scalp with slightly warmed sesame oil. This relieves tension headache and induces sleep.

Regular exercise, yoga and meditation reduce the stress levels and keep the tension headache at bay.

Persons of vata type can use hot water bags on aching parts and a hot shower will be of good help.

Persons of pitta type can use cold or ice packs on aching parts and wrap a wet cold cloth around neck.

Home remedies for migraine headache.

Make a note of facts which trigger the headache and also the foods. Try to avoid these . Avoid spicy foods, coffee, tea, alcohol, onion and garlic. Avoid citrus fruits except lemon.

Try muscle relaxation exercises. Progressive muscle relaxation, meditation and yoga are definitely going to help. Relax at least for half an hour everyday by doing your favorite activities like listening to music, gardening, taking hot water bath etc.

Stick to you a regular sleep pattern and get enough sleep but don't oversleep. Avoid sleeping during the afternoon.

Rest in a dark, quiet room when you feel a headache coming on. Place an ice pack wrapped in a cloth on the back of your neck and apply gentle pressure to painful areas on your scalp.

Home remedies for sinus headache.

• Breathe in warm, moist air.

• Moist the nasal passages with the help of saline nasal spray. A saline spray can be used up to six times per day.

• Do not smoke. If you smoke, quit.

• Avoid second-hand smoke and polluted air.

• Avoid exposure to anything that triggers allergy or sinus symptoms.

• Wash your hands frequently to avoid colds.

It is no wonder that every urbanite's cupboard or medicine cabinet is often stocked with pain relief medications. When the headache comes back, we simply repeat the practice. While it seems harmless to take headache pills for a day or two, overusing it can have unexpected and serious consequences.
Overusing pain relief medications can become a cycle. It can make headaches return as soon as the effect of the medication wears off. Once the pain returns, the tendency is to take more medications until it becomes a worse cycle. Medication overuse headaches are also known as rebound headaches . Rebound headaches are caused by medication taken to relieve regular headaches. In the case of problem headaches, such medication can be a lifesaver. However, the overuse of certain pain relief medications can cause headaches to occur more often and become more severe. People should learn to understand this problem and develop ways to deal with it in order to take control of frequent headaches.
Rebound headaches occur when pain relief medication is taken too frequently to relieve headache pain. It's not easy to diagnose rebound headaches. Overuse is determined by asking patients if their headaches assumed a new pattern or became more severe after taking medications excessively. More than three times a week is generally considered excessive medication. However, to make a sure diagnosis of rebound headache, the patient must be withdrawn from medication anywhere up to 6 times.
Breaking this cycle means discontinuing the use of the medication. Bear in mind that such withdrawal will actually heighten the headache for the first few weeks. Withdrawal may not be easy. It may take days, weeks, or even months of agonizing withdrawal symptoms, which include headaches and fatigue. Smith, the medical director of the Ryan Headache Center in St. Louis, many experts still don't know exactly what causes rebound headaches, though the regular overuse of pain relief medication can cause physiological changes. The use of too much pain relievers can lower a person's pain threshold so that they begin to require more painkillers just to feel “normal” again.
Pain reliever overuse can actually lower the level of serotonin, a hormone that acts as a chemical messenger which transmits nerve signals between nerve cells and causes blood vessels to narrow. Changes in the serotonin levels in the brain can alter the mood as well as pain perception.
The problem with rebound headaches is that they can be difficult to identify. People with rebound headaches usually had chronic headaches to begin with, which is precisely the reason why they started to take medication in the first place. It would be difficult for both the doctor and the patient to notice the shift from a migraine headache to a rebound headache.
Symptoms, however, can vary from person to person. The nausea and sensitivity to light that are typically present with migraine headaches are usually absent in rebound headaches, and the pain can felt in different parts of the head. Patients with rebound headaches will usually complain of daily headaches with pain that escalate to a point where it's interfering with normal lifestyle, causing anxiety and depression to the individual. According to experts, any pain relief medication is capable of causing rebound headaches when taken frequently and in more than enough dosage. Even over-the-counter medications that contain aspirin, acetaminophen, and ibuprofen can also cause rebound headaches. Chiropractor Says Space-Age Technology Provides Down-To-Earth Benefits For Patients

What does the Russian space program have to do with chiropractic care? Quite a bit, according to Tom Lenahan, DC.

Dr. Lenahan, who has practiced chiropractic as a second career for 15 years, says he’s seen tremendous breakthroughs in patient healing through the use of electrical stimulation—a concept first researched by the Russians for their space program. The Russian government developed the research because it wasn’t practical to send an entire pharmacy along to keep their space explorers healthy. So, Russian scientists started looking for healing alternatives.

“As they began to explore how electricity works in the body, they realized it’s the body’s primary driver system,” says Lenahan. The researchers found that the body has two electrical components. The first, the nervous system, is like the hard wiring, says Dr. Lenahan. The second wasn’t as well understood.

“They discovered the body has a second component to it,” Lenahan continues. “It is a semi-conducting system in the body, much like your computer. They realized they could stimulate that fascial, semi-conducting system to send a signal to the brain and get it to produce various neuropeptides. This gave them an effective, efficient means of treatment for anything they would run into in space. It’s still one of the primary components of the Russian battlefield first-aid kit.”

It might still sound like a strange fit with chiropractic techniques, but Lenahan insists it isn’t. In fact, Lenahan discovered the power of electrical stimulation in his own quest to find better ways to help his chiropractic patients.

“As I got into chiropractic school, I realized there were many facets that were needed to help the body heal,” says Dr. Lenahan. “In our profession, there are those who are ‘straights’ and those who are ‘mixers.’ I’m the mixer’s mixer. I’ve always sought avenues and technology that would benefit my patients.”

That’s why he was so excited when he found the technology of electrical stimulation. He’s so enthusiastic about it, he speaks on the topic at numerous seminars, including the upcoming International Chiropractic Appreciation Mega Event (ICAME) in Las Vegas in January. While there, he says he’ll give an in-depth scientific presentation on this startling technology.

“It works for acute injuries, but also for the chronic degenerative condition,” he says. “This technology is able to add energy into the system and retard that degenerative process.”

Not only will this technique allow chiropractors to help their patients in ways never thought possible, says Lenahan, but it will also result in a boost in chiropractic practices.

“The…units will let you address really severe pain. They will help you alleviate that and get your patients back in, and keep them coming in,” Dr. Lenahan explains.

“A lot of times we lose patients because the pain is so severe, and they feel they have to go someplace and get drugs. This will help you manage that type of process.”

Third party payers demand documentation to substantiate the medical necessity of a chiropractic treatment. This documentation should support care by providing evidence that a treatment was necessary for relief from a condition and to alleviate a patient’s symptoms. When a third party payer requests records, there are necessary documents and information that must be included for review. These include a patient’s past and present history, consultation form, chart notes and any required x-rays.
The patient’s past and present history are needed. These must explain why the patient sought chiropractic care, the nature of the complaints, the duration of the complaints and whether the patient was involved in a traumatic event. The documentation should explain if the patient has exhibited the same or similar complaints previously, whether there is a family history of illness or injury and if there are contraindications to chiropractic care.
It is important that the chiropractor include a consultation form. This reiterates the reasons the patient sought chiropractic care. The form should explain any previous medical and chiropractic treatment, as well as any prior examinations and test results, if known. This indicates whether patient’s condition is acute or chronic and what treatment, has been provided to the patient, along with the prior treatments outcome. Diagnoses are necessary.
The initial examination findings are important. These findings should describe range of motion, orthopedic and neurological, chiropractic palpation and postural abnormalities. If a home program was instituted, they tell whether the patient is taking charge of his or her own healthcare and if it appears that the patient’s condition is progressing or being alleviated.

Nearly 30 million Americans suffer from migraine headaches. Migraines occur more commonly in women than in men and typically occur in people aged 15 to 55 years.

A recent study of 171 migraine sufferers, published in the journal Headache, suggests that over-the-counter Excedrin Migraine works better the prescription Imitrex (sumatriptan) for treating migraines. The patients who took the Excedrin Migraine at the onset of a headache experienced less pain after four hours than those who took Imitrex.

This result is probably true for some patients but not all. Excedrin Migraine contains acetaminophen, aspirin, and caffeine. The problem with continual use of this combination is that it often leads to rebound headaches. You should also not take more than two tablets per day and no more than twice per week. This greatly reduces your chances of rebound headaches. If you are experiencing more than two migraines a week, you will need a prescription medication.

Another study found that taking an NSAID, like naproxen, with Imitrex (sumatriptan) works much better than taking either medication alone for migraine headaches. The researchers concluded that the best combination is 50mg of Imitrex (sumatriptan) combined with 500mg of naproxen.

Imitrex (sumatriptan) is in a class of medications called triptans. There are seven triptans currently available in the United States. They are Imitrex (sumatriptan), Maxalt (rizatriptan), Zomig (zolmitriptan), Amerge (naratriptan), Frova (frovatriptan), Relpax (eletriptan), and Axert (almotriptan). Triptans are used only for migraine treatment because they are effective only at relieving the pain caused by migraines. They are slightly useful for some other headache types but not for all headache pain or for any other type of pain.

The current guidelines for migraine headaches state that NSAIDS are first line therapy. That means they should be used first. If someone is allergic, or the NSAIDS do not work, then the triptans are the drugs of choice.
Patients who suffer from migraine headaches should have a treatment plan in the event they get a migraine. This plan should include both preventive medication and medications to stop the migraine. The first line medications, NSAIDs and triptans, are the primary choices.

This study suggests that the NSAID combination of acetaminophen, aspirin, and caffeine is more effective than oral sumatriptan at relieving migraine headache pain, when taken at the onset. More studies should be conducted to confirm these results since patients with headaches that cause vomiting or require the patient to lie down were not studied. This study did confirm that both the combination of acetaminophen, aspirin, and caffeine and triptans appear to be effective first-line therapies for patients with migraine headaches.

I would recommend using a store or generic brand headache medication containing 250mg of acetaminophen, 250mg of aspirin, and 65mg caffeine first, for mild migraines. This combination costs about 4¢ per pill. If this is ineffective, or the migraine is severe, move up to the triptans. They are expensive at about $17.00 per pill. Anyone with a migraine will tell you that cost is not a consideration when it comes to migraine relief.

The same heat herbal pack can be put in the freezer and used as a cold herbal pack and treat inflammation.

When considering back pain we must concern ourselves with its variants. For instance, back pain can start with slip disks, which in medical terms is called “Herniated nucleus pulposa.” (HNP) Doctors define slip disks as ruptures of the “intervertebral disk.” The intervertebral rests between the vertebrae (Spinal Column) of the backbone.

The interruption has variants, including the “Lumbrosacral,” (L4 and L5) as well as cervical C5-7. The cervical is at the neck and belongs to other parts of the back and neck as well. Among the different causes of injuries among children and adults, car accidents have been reported as one of the top causes. It accounts for 55% of spinal injury cases in the US.

What kinds of injuries can I get from car accidents?

Car accident injuries affect different parts of the body. The most commonly affected are the head, neck and spine, and at time some other organs of the body, too.. But car accidents also frequently affect internal organs, and upper and lower limbs. The three most common injuries would be the whiplash, traumatic brain injury, and back injury. Other injuries may include those of the limbs and internal organs.

Whiplash. The most common type of car accident back injury is the whiplash. It accounts for more than one million car accident back injuries each year. Ten percent of these car accident back injuries lead to long term disability. This type of car accident back injury occurs most often during rear end collisions and can also when the car strikes an immobile object or gets hit broad-side by another vehicle. Traumatic Brain Injury. Traumatic brain injury (TBI) is a really serious injury that may result in a car accident. This occurs when the external force of a car accident traumatizes the brain. It may lead to permanent or temporary impairment of the neurological functions. Skull fractures, bruises of the brain (hematoma), and nerve damage are often related to such brain injury. Brain injuries are more common in side impact car accidents than in rear end auto accidents. Brain damage in a car accident is often related to the quick acceleration and deceleration of the brain, which causes injury to the point of impact and its opposite point.
Back Injuries. Thoracic disks and lumbar disks may also be injured in car accidents. Compression fractures may be caused in such accidents which can cause permanent disability. Also, herniations or bulges may cause spinal cord compression. Symptoms of spinal injuries include arm and/or leg weakness, paralysis, difficulty breathing, numbness, tingling, and abnormal bowel or bladder control. The back nerves, ligaments and muscles may also be traumatized by a car accident, causing pain and inflammation.
Internal Organ Damage. Internal injuries from a car accident may include injuries to bowels, kidneys, the spleen, liver, lungs, heart or aorta. Fractured ribs are quite common and may also puncture lungs and other internal organs. Torn spleens are also a frequent injury.
Upper and Lower Limbs. Car accidents may cause minor or severe fractures to your hands, forearms, arms, shoulders, wrists, fingers, hips, legs, knees, heels, ankles, and feet if the impact has caused sudden pressure to the bones.

How can we treat these injuries?
Any car accident injury, if not fatal, may be cured by constant therapy and proper medications. Disc injuries may require surgery to remove disc material fragments or spinal fusion. Prescriptive medications are often used initially but for continuing pain and inflammation, patients are sent to physical therapists or chiropractors to rehabilitate motor functions and relieve the pain. Therapy generally includes infrared heat, hot packs, paraffin bath, hydrotherapy, diathermy, cold packs, ultrasound, electrical stimulation, muscle strengthening, traction, massage, or acupuncture. Another common treatment is steroid injection. Severe injuries may require hospitalization.

Most people are unaware that the use of Botox for the prevention of migraine headaches came about purely by accident. It all happened through the use of Botox by plastic surgeons. Patients undergoing plastic surgery, who were also experiencing headaches, noticed that their injections of Botox helped with their migraine headaches.

Botox, also known as botulinum toxin, is a neurotoxin (a toxin that attacks nerve cells). It was originally approved by the FDA for use in conditions where hyperactivity of muscles was an underlying problem of related health and medical conditions. The theory is that Botox weakens or paralyzes muscles by inhibiting the release of acetylcholine. Acetylcholine is an ester of choline that is thought to play an important role in the transmission of nerve impulses at synapses and myoneural (muscle-nerve) junctions. By inhibiting the release of acetylcholine the nerves cannot cause a muscle contraction, thus relaxing the muscle. Botox is administered by being directly injected into the affected muscles.

There are differing theories as to how Botox works to eliminate migraine headaches, but the exact reasoning remains unclear. One theory is that muscle tension and spasms are a cause of migraines and by eliminating the muscle tension with an injection of Botox it will also eliminate the cause of the headaches. If given Botox before the headache begins it is possible to eliminate the headache before it even starts.

When muscles spasm it can irritate nerves that innervate them. When these nerves send signals to the brain in an increased amount it causes pain and possibly headaches. Botox may affect these nerve cells, inhibiting their transmission, causing a lessening of the perception of pain. Many researches think that migraine headaches are a result of inflammation of blood vessels. However, muscle tension may also play an important role in the cause of headaches.

When researchers test a medication's effectiveness they usually test it through a double-blind study. This means that some patients are given the medication being tested while others are given a placebo. A placebo is an inactive substance, such as sugar, that is given in place of the medication that is being evaluated. Some tests have been performed that did not involve double-blind studies, but showed that Botox could prevent migraine headaches. One double-blind study involving Botox and headaches showed only a small positive effect.

Initial studies may not have shown the full effectiveness of Botox for the prevention of headaches. The injections were not placed in the typical locations of the headache pain, but in areas normally injected during plastic surgery. In subsequent studies, using larger doses and injecting into more specific localities for the headache pain, the results were improved. These tests were concerning migraine headaches. However, with tension headaches, or muscle tension headaches, the studies have shown improved results. This is to be expected due to the weakening or paralyzing nature of Botox on the muscles that are experiencing tension.

When being injected with Botox for the prevention of headaches it may take several weeks before the injection takes effect. For this reason it is usually necessary to have it injected no more often than once every 3 months or so. People that have Botox administered for the treatment of headaches typically need it injected every few months. If there is too long of an interval between injections the headaches may be experienced by the patient.

However, the question arises about the safety of these regular injections of a neurotoxin. Since 1989, when it was first used as a headache treatment, it has appeared to be fairly safe. Most migraine headache medications are taken orally and eventually end up in the blood stream. Because of this common symptoms of migraine headache medication will be dizziness and drowsiness among other symptoms. Since Botox is injected directly into the muscle tissue and does not get absorbed into the blood system as does the typical headache medicine there are fewer side effects. Most commonly, the only side effects of injecting this neurotoxin is pain in the area of injection for a couple of days or drooping of the eyelids for a few days.

Studies are still being performed on this medication and as yet the FDA has not approved it for the treatment of headaches. However, physicians do prescribe it to their patients and if you think you are a candidate to use it you should discuss it with your doctor. If you do not respond to other headache medications it may be something to talk with your physician about. As the studies continue the effects and safety issues involving this medication should become more clear.
