A migraine aura is a specific type of prodrome, or premonition, that heralds a migraine. It is a symptom or series of symptoms that let migraineurs, people who suffer from chronic migraines, know in advance that a migraine headache is about to make itself felt.

What actually happens in your body to cause headache?

The skull bones and brain tissues do not have pain sensitive nerve fibers. But muscles of face, mouth, throat, neck, scalp and blood vessels on the surface and base of brain are sensitive to pain as they contain pain sensitive nerve fibers.

Causes like less sleep, stress, reading for a long time, and working in front of a computer for long hours can precipitate tension headache.

Migraine Headache: Migraine headache is caused when blood vessels of head get too much dilated or constricted. Females are more prone to this type of headache than males and migraine tends to run in families.

A person suffering from migraine headache feels like vomiting becomes too sensitive to light, sound and other activities. Few people experience spots or flashes before headache.

Few facts like menstruation in females, caffeine, alcohol, aged cheese, cured meat, stress, change in sleeping pattern, strenuous exercise trigger migraine headache.

Sinus Headaches

A sinus headache surfaces when fluids in the nose are not able to drain well and pressure builds up in sinuses. A cold, dust allergies, dirty or polluted water air travel may cause this type of headache.

Getting a headache has become all-too-natural for people who live hectic, hurried lives in the city. It is no wonder that every urbanite's cupboard or medicine cabinet is often stocked with pain relief medications. When the headache comes back, we simply repeat the practice. While it seems harmless to take headache pills for a day or two, overusing it can have unexpected and serious consequences.
Overusing pain relief medications can become a cycle. It can make headaches return as soon as the effect of the medication wears off. Once the pain returns, the tendency is to take more medications until it becomes a worse cycle. Medication overuse headaches are also known as rebound headaches . Rebound headaches are caused by medication taken to relieve regular headaches. In the case of problem headaches, such medication can be a lifesaver. However, the overuse of certain pain relief medications can cause headaches to occur more often and become more severe. People should learn to understand this problem and develop ways to deal with it in order to take control of frequent headaches.
Rebound headaches occur when pain relief medication is taken too frequently to relieve headache pain. It's not easy to diagnose rebound headaches. Overuse is determined by asking patients if their headaches assumed a new pattern or became more severe after taking medications excessively. More than three times a week is generally considered excessive medication. However, to make a sure diagnosis of rebound headache, the patient must be withdrawn from medication anywhere up to 6 times.

Third party payers demand documentation to substantiate the medical necessity of a chiropractic treatment. This documentation should support care by providing evidence that a treatment was necessary for relief from a condition and to alleviate a patient’s symptoms. When a third party payer requests records, there are necessary documents and information that must be included for review. These include a patient’s past and present history, consultation form, chart notes and any required x-rays.
The patient’s past and present history are needed. Nearly 30 million Americans suffer from migraine headaches. Migraines occur more commonly in women than in men and typically occur in people aged 15 to 55 years.

A recent study of 171 migraine sufferers, published in the journal Headache, suggests that over-the-counter Excedrin Migraine works better the prescription Imitrex (sumatriptan) for treating migraines. The patients who took the Excedrin Migraine at the onset of a headache experienced less pain after four hours than those who took Imitrex.

This result is probably true for some patients but not all. Excedrin Migraine contains acetaminophen, aspirin, and caffeine. The problem with continual use of this combination is that it often leads to rebound headaches. You should also not take more than two tablets per day and no more than twice per week. This greatly reduces your chances of rebound headaches. If you are experiencing more than two migraines a week, you will need a prescription medication.

Another study found that taking an NSAID, like naproxen, with Imitrex (sumatriptan) works much better than taking either medication alone for migraine headaches. The researchers concluded that the best combination is 50mg of Imitrex (sumatriptan) combined with 500mg of naproxen.

Imitrex (sumatriptan) is in a class of medications called triptans. There are seven triptans currently available in the United States. They are Imitrex (sumatriptan), Maxalt (rizatriptan), Zomig (zolmitriptan), Amerge (naratriptan), Frova (frovatriptan), Relpax (eletriptan), and Axert (almotriptan). Triptans are used only for migraine treatment because they are effective only at relieving the pain caused by migraines. They are slightly useful for some other headache types but not for all headache pain or for any other type of pain.

The current guidelines for migraine headaches state that NSAIDS are first line therapy. That means they should be used first. If someone is allergic, or the NSAIDS do not work, then the triptans are the drugs of choice.
Patients who suffer from migraine headaches should have a treatment plan in the event they get a migraine. This plan should include both preventive medication and medications to stop the migraine. The first line medications, NSAIDs and triptans, are the primary choices.

This study suggests that the NSAID combination of acetaminophen, aspirin, and caffeine is more effective than oral sumatriptan at relieving migraine headache pain, when taken at the onset. More studies should be conducted to confirm these results since patients with headaches that cause vomiting or require the patient to lie down were not studied. This study did confirm that both the combination of acetaminophen, aspirin, and caffeine and triptans appear to be effective first-line therapies for patients with migraine headaches.

I would recommend using a store or generic brand headache medication containing 250mg of acetaminophen, 250mg of aspirin, and 65mg caffeine first, for mild migraines. This combination costs about 4¢ per pill. If this is ineffective, or the migraine is severe, move up to the triptans. They are expensive at about $17.00 per pill. Anyone with a migraine will tell you that cost is not a consideration when it comes to migraine relief.

When considering back pain we must concern ourselves with its variants. For instance, back pain can start with slip disks, which in medical terms is called “Herniated nucleus pulposa.” (HNP) Doctors define slip disks as ruptures of the “intervertebral disk.” The intervertebral rests between the vertebrae (Spinal Column) of the backbone.

The interruption has variants, including the “Lumbrosacral,” (L4 and L5) as well as cervical C5-7. The cervical is at the neck and belongs to other parts of the back and neck as well. Backpain and ESI are related to each other in that the latter is very often used to find out the true cause of the former. Back ache is a common phenomenon and is generally a result of a strain or injury to the muscles and ligaments supporting the human spine. An Epidural Steroid Injection or ESI is the most common type of injection given to patients suffering from back and neck pain. The pain in the back can be constant or intermittent and its intensity may vary from a dull ache to a searing feeling.

In a large number of cases, back pain vanishes or can be cured within a few weeks. However, in cases of chronic pain, medical advice is necessary. Patients having difficulty in passing urine or experiencing numbness in the back or genital area or weakness and pain in the legs or feeling unsteady while standing should see a physician immediately. Spinal injections are often used to find out the true cause of the back pain and than to actually treat the pain. Injections which provide temporary relief are called therapeutic injections. In other cases spinal injections are used to diagnose the cause of the back pain. ESI is generally injection used to treat back pain.

The space between the covering of the spinal cord and the inside of the bony spinal canal is called the epidural space. An injection in this area ensures that the medication moves across the whole of the spine and covers all the nerve routs and joints to relieve the back pain. ESIs are generally very effective in the treatment and the true diagnosis of the back pain.

In most of the cases where spinal injections are used, a local anesthetic called Lidocaine is used. Lidocaine is a fast acting drug and its effects wear off in a couple of hours. So it is generally used as a diagnostic tool. Bupivacaine, another anesthetic is used to provide some relief from the back pain. Another commonly used injection is Cortisone. Cortisone is commonly injected with a local anesthetic to reduce inflammation in the affected areas of the patient suffering from severe back pain.

An epidural steroid injection or ESI can be injected in three different ways. In the first case a caudal block is placed through the sacral gap which is the space below our lumbar spine. Than the injection is placed into the epidural space. This type of block reduces the chances of puncturing the dura.

The most common method of giving an ESI to reduce back pain is the Tran lumbar approach. Under this method the needle is placed between vertebrae from the back. The third method is generally used for diagnostic purposes. The transforaminal approach involves injection of the medication around a specific nerve root to find out the exact problem area. The foraminae are small openings between your vertebrae through which the nerve roots exit the spinal canal and enter the body.

Among the different causes of injuries among children and adults, car accidents have been reported as one of the top causes. It accounts for 55% of spinal injury cases in the US.

What kinds of injuries can I get from car accidents?

Car accident injuries affect different parts of the body. The most commonly affected are the head, neck and spine, and at time some other organs of the body, too.. But car accidents also frequently affect internal organs, and upper and lower limbs. The three most common injuries would be the whiplash, traumatic brain injury, and back injury. Other injuries may include those of the limbs and internal organs.

Whiplash. The most common type of car accident back injury is the whiplash. It accounts for more than one million car accident back injuries each year. It may lead to permanent or temporary impairment of the neurological functions. Skull fractures, bruises of the brain (hematoma), and nerve damage are often related to such brain injury. Brain injuries are more common in side impact car accidents than in rear end auto accidents. Brain damage in a car accident is often related to the quick acceleration and deceleration of the brain, which causes injury to the point of impact and its opposite point.
Back Injuries. Thoracic disks and lumbar disks may also be injured in car accidents. Compression fractures may be caused in such accidents which can cause permanent disability. Also, herniations or bulges may cause spinal cord compression. Symptoms of spinal injuries include arm and/or leg weakness, paralysis, difficulty breathing, numbness, tingling, and abnormal bowel or bladder control. The back nerves, ligaments and muscles may also be traumatized by a car accident, causing pain and inflammation.
Internal Organ Damage. Internal injuries from a car accident may include injuries to bowels, kidneys, the spleen, liver, lungs, heart or aorta. Fractured ribs are quite common and may also puncture lungs and other internal organs. Torn spleens are also a frequent injury.
Upper and Lower Limbs. Car accidents may cause minor or severe fractures to your hands, forearms, arms, shoulders, wrists, fingers, hips, legs, knees, heels, ankles, and feet if the impact has caused sudden pressure to the bones.

How can we treat these injuries?
Any car accident injury, if not fatal, may be cured by constant therapy and proper medications. Disc injuries may require surgery to remove disc material fragments or spinal fusion. Prescriptive medications are often used initially but for continuing pain and inflammation, patients are sent to physical therapists or chiropractors to rehabilitate motor functions and relieve the pain. Therapy generally includes infrared heat, hot packs, paraffin bath, hydrotherapy, diathermy, cold packs, ultrasound, electrical stimulation, muscle strengthening, traction, massage, or acupuncture. Another common treatment is steroid injection. Severe injuries may require hospitalization.

Why Do Injuries Occur In Golf?

Injuries occur in all athletic events quite frequently, certain sports more so than others. Golf is no different than any other sport. The severity of injuries in golf usually are not as severe as in other sports. The scenario of a 300 lb. defensive lineman slamming into the side of your knee tearing every possible ligament structure in the knee will never happen in the sport of golf. An interesting visual if you combined the sports of football and golf onto the same playing field, but inappropriate for this paper.

There are two types of injuries classified by professionals in the fields of athletic training and sports medicine. The two types of injuries are: 1) acute and 2) chronic. The above example of the football player is classified as an acute injury. An acute injury can be defined as the trauma in the body occurring immediately after the injury. Refer to the football player example above for a reminder. (For us older golfers, remember Joe Theisman of the Redskins and Lawrence Taylor’s leg breaking tackle? Acute injury.) Relating an acute injury to golf is a little more difficult. Probably the easiest, and maybe most the common, acute injury in golf, occurs while swinging and you hit a rock or something that creates an injury to your wrist. That would be the best example in the sport of golf of an acute injury. Overall, acute injuries tend to be rare in golf because contact by the body with external forces is rare.

My back is always killing me!

The second type of injury, chronic, is much more prevalent when it comes to the sport of golf. A chronic injury is one that occurs over time. Think of it as a “wear and tear” injury. These are usually the result of the body breaking down over time. A great sports example outside of golf is when you hear about a baseball pitcher having tendonitis in the elbow. Tendonitis is an inflammation of the elbow resulting from the stresses placed upon it from throwing. Over time the elbow becomes tired and eventually injured from the number of pitches thrown. If you are a runner and, after a certain amount of time, your knees begin to hurt, this is usually a chronic injury. When we talk about golf, the majority of injuries are chronic. They tend to be a direct result of the golf swing (just like the pitcher’s elbow). Usually the chronic injuries in golf show up in the lower back. If chronic injuries are caught soon enough in the cycle, rest and proper treatment (i.e. massage, chiropractic care) will heal them. But if you wait too long the body is going to “break,” and then you will not be playing any golf for a long time. This is where the unfortunate situation of surgery and other invasive procedures are considered.

So a couple of questions we must ask when it comes to chronic injuries in relation to golf are: how do they occur, and how do we prevent them? Chronic injuries occur as a result of the body becoming fatigued and eventually “breaking down.” The muscles, ligaments, and tendons of your body are required to perform the activity of swinging a golf club. Over time this activity causes fatigue within your body. As the body continues to fatigue, or get tired, the body gets sore. This is the first indicator of a developing chronic injury. If you continue with the activity you’re participating in, with soreness in the body, eventually your body will break down. This “break down” will be in the form of maybe a pulled muscle, muscle stiffness, tightness, or some other type of inflammation. All of the above examples are a result of structures in your body breaking down from fatigue and overuse. Even if just on one swing you feel “your back go out,” nine out of ten times it is a chronic injury, and that last swing was the “piece of straw that broke the camel’s back.”

How to Prevent Chronic Injuries in Golf

We all know that the golf swing is a repetitive movement, meaning the body is performing the same activity over and over again. This creates fatigue in the body over time. And if over time our body can’t support the number of swings we are taking, it is eventually going to break down. There are three variables we have when it comes to the prevention of chronic injuries in golf. Number one is workloads. Workloads can be defined as the number of swings that the body takes with a club over a given period of time. That time frame can be seven days or an entire tour season. Number two is efficiency of your mechanics. When we say “efficiency of mechanics” we are talking about how biomechanically correct your individual swing is. “Why is this important?” you ask. Let me tell you. I think most of us would agree that the tour players have very “efficient” swings; their swings are smooth and look almost effortless. A swing like this asks less out of the body to perform and requires less effort from the muscles; hence fatiguing levels in the body are lower. Some amateur swings look like they take a lot of work to perform, and in reality they do! These types of swings ask a lot more out of the body and fatigue it more quickly. The final variable is what we term “golf strength.” Golf strength is a measure of the required levels of flexibility, strength, endurance, balance, and power to successfully support the mechanics of the swing. Large amounts of golf strength allow the body to support an efficient swing. Low levels of golf strength do not provide the support needed for the swing.

Workloads, Swing Mechanics, and Golf Strength

All three of these variables work together to determine if you are a candidate for a chronic golf injury. Golf strength is essentially the foundation upon which your swing is built. This variable indicates how many times you can swing a golf club with your current mechanics before you come up injured. If you have high levels of golf strength then regardless of how efficient of a swing you have, you will be able to play for quite awhile before you get sore. The flip side can also be said. If you have low levels of golf strength, regardless of your swing mechanics, you will come up sore in a shorter amount of time.

Secondly, let us look at swing mechanics. If you are a player that has a very efficient swing that places very little stress on the body, you will undoubtedly be able to play many rounds before your body starts screaming at you. Again, if you have poor mechanics, it is going to take its toll on your body and your game.

Finally, we have workloads (i.e. number of swings). The number of swings one makes must match up with levels of golf strength and swing mechanics. The golf swing is a “stressor” of the body and breaks it down over time. If you have an efficient swing, each swing does less “damage” to the body. If you have a poor swing, the body has to work harder, thus fatiguing it more quickly. In addition to this is golf strength. If you have high levels of golf strength, you can swing the club more (i.e. workloads) before you get tired. Low levels of golf strength present the situation of the body’s fatiguing more quickly. “So what is the magic formula?” you ask. My first suggestion is twofold: 1) work on your swing to improve the efficiency of it, and 2) increase your levels of golf strength in order to support your swing. For the time being, match up your swing and golf strength levels to determine what workload levels you can get out of your body.
